What is an Elder Care Manager?

An Elder Care Manager is a health and human services professional, such as a nurse, social worker or gerontologist. They specialize and focus on issues related to aging and elder care. They know the resources available to you and your loved one while going through the aging process. Our professional care managers can provide you with peace of mind regarding the care for your loved ones. Each one is trained in geriatric care or mental health services and is licensed as an RN or MSW.

How can an Elder Care Manager help?

Elder Care Managers work privately with older adults and their families to create a plan of care which meets the needs of the client and their family. We understand that each situation is unique and different and we tailor your care plan and advice to your situation. Having our staff provide consultation and oversight can give you peace of mind and, we are a neutral party so we can handle difficult family dynamics.

What does an Elder Care Manager do?

An Elder Care Manager can handle anything from a quick telephone consultation to a complete assessment with a written care plan and then provide ongoing supervision and monitoring. Basically, they conduct care assessments and needs, identify problems, propose solutions and can even help with ongoing monitoring and supervision.

How much does a Care Manager know?

We work in a team environment, meeting weekly to discuss every client. The approach brings many disciplines and backgrounds to arrive at the best decision. We have extensive knowledge about the resources, quality, costs and availability of services in the community. As our families start the journey of aging and need long term care, we will be there as an advocate and to provide support to accompany them along the path.

Can they help find the "Right" solution?

Most individuals wish to remain in their own homes for as long as safely possible. We can make the determination if it is the right time to move your loved one to a home and if so, help make sure the "right" facility is chosen. One of the most common and costly mistakes made is to move your loved one into a facility which does not meet their needs. If your loved one is able to remain safely in their home, we can help you select the right in-home services to make certain they are comfortable and remain independent.

Who pays for the Elder Care Manager?

Most times, the family or the client will pay privately for the services. We do not accept referral fees from facilities or home health agencies. Doing so would impair our independent and objective evaluation. In some instances, our services can be paid for through long term care insurance although it varies from policy to policy. If you or your loved one has long term care insurance, please consult with your insurance company.

How long will the initial assessment take?

The initial assessment timing will depend on the complexities of each case. Typically, we can complete the initial assessment in 3-7 business days although we can work emergency situations much quicker.

How much will your services cost?

Our costs range depending on the time and amount of services required. All services are billed on an hourly basis in 15 minute increments with rates varying, depending on the depth and amount of services needed. Professional charges vary depending on who performs the service and what type of service is provided. A rate sheet will be provided on the initial visit. We accept Visa, Mastercard and personal checks.

Why are other Referrals or Case Managers free?

Many times, professionals in our industry accept referral fees or commissions from home agencies or facilities, providing the advice for "free". We strongly believe the practice impairs their judgment and in the cliché "you get what you pay for".

Can I stop service anytime?

Yes, all we require is a formal notification, such as a phone call, from a family member with the proper authority.

Do you only deal with older clients?

No, we provide services for all ages that require assistance and care management such as developmentally or physically disabled or injured individuals.

Aren't the same services provided by Discharge Planners, Skilled Nursing or Hospitals?

No, although others may address a few areas of health care, we provide an ongoing single point of contact to help manage care for a long time. Also, others do not provide a care plan to help with ongoing support.

Do you provide a written report?

Generally yes, depending on the depth and type of services you have requested.

Can I hire you for a specific issue?

Yes. Honoring Elders can be retained for a specific service, length of time or for limited involvement. We are here to help you in whatever care management need you may have.
