We can assess your loved one, identify problems or concerns and help you solve the issues by providing a Plan of Care.
Independent, objective assessment of the ability to remain at home
We are trained to objectively assess each situation, listening carefully to the needs of both seniors and family members and help arrange in-home care if needed.
Identification and Monitoring of In-Home Care
If your loved one is able to remain at home, we can help select and train caregivers and/or monitor your selected home care agency. This provides you with confidence your loved one's best interests are being watched.
Identification of the best facility for your loved one's needs
If it is clear your loved one cannot safely remain at home, we help identify the right facility for his or her needs - both short and long-term.
Short-Term or Continuous Care Management
Working closely with physicians and medical advisors, we can assist in the health care management of your loved one on a short-term or long-term basis, providing a single point of contact between all parties, stepping in and providing support and advice when needed.
